Referrals can be made to individual specialists or to the practice via fax, Healthlink or eMail.

The attached referral form can be printed and completed, or completed electronically and sent by fax or email.

Please indicate on the referral form whether the patient has had any imaging performed and provide the name of the imaging provider (e.g. Rad X, Qscan, etc.).
